Cancellation Fees:
     $ (15 - 21 days prior: 15% of honorarium)
     $ (8 - 14 days prior: 30% of honorarium)
     $ (0 - 7 days prior: 50% of honorarium)
If, for any reason, the appearance is canceled from 15 -21 days prior to the appearance date a 15 % cancellation fee will be payable at time of cancellation. If cancellation is made from 8 - 14 days prior to appearance date a 30% fee will be payable. If cancellation is made from 0 - 7 days prior to appearance date a 50% fee will be payable.

The performer's props may not be touched at any time without full consent and supervision of the performer. Any delay in starting time of 30 minutes or more without previous discussion with the performer is considered a breech in this agreement.

Approximately 40 feet needs to be provided for the performer's vehicle and trailer, with easy access to parking and the performance area for set up and tear down. Please provide 3-4 men for moving in and out equipment before and
after 60 minute presentations. Set up & tear down will take approximately 2 hours each.

This agreement is entered into by the undersigned parties in a spirit of Christian brotherhood for the purpose of establishing good communication and mutual understanding. By clicking on the box at the bottom of this form, the sponsor's acknowledges agreement with all information written on this form, as well as any attached agenda. This agreement is not confirmed until it is approved by Darren Smith with the agreed upon deposit. Must call and confirm date and time one week before performance.
